Responsibilities- Responsible for coordinating the care and service of patients across the continuum promoting quality care, effective utilization of resources and assuming a leadership role with the interdisciplinary team to achieve desired clinical and resource outcomes.
- This is accomplished by assessing, planning, implementing, and evaluating the delivery of services and working collaboratively with all members of the health care team.
- Plans and coordinates a comprehensive plan of care using a concurrent integrated review, standard operating procedures, case management tools, and collaborative efforts with all members of the patient's family and/or significant other to enhance quality care and reduce costs.
- Coordinates patient care to ensure appropriate and effective clinical outcomes.
- Communicates with third party payors, peer review entities and physicians' offices to gather and provide clinical and fiscal information for financial optimization.
- Contributes to a positive work environment and performs other duties as assigned or directed in order to enhance the overall team efforts of the department/organization.
Qualifications Job Requirements:
- Active Registered Nurse License in state of Texas
- 3 years of hospital experience required
Preferred Job Requirements:
- Previous case management experience preferred
